What Is an Explanation of Benefits (EOB)?

An Explanation of Benefits (EOB) is a document sent by your health insurance company after a medical claim is processed. It explains what was billed, what your insurer paid, and what you may owe.

Important: An EOB is NOT a bill. It's a summary of how your claim was processed. You'll receive a separate bill from your provider.

What an EOB Shows

• The provider's name and service date
• What was billed (the full charge)
• The negotiated/allowed amount (what your insurer pays as maximum)
• What your plan paid
• What counts toward your deductible
• Your patient responsibility (what you owe)

EOBs are essential for tracking your annual deductible and out-of-pocket maximum progress. Keep them to verify that your insurer has correctly applied payments to your deductible.
